Normally, the atria contract before the ventricles. This allows the atria to empty into the ventricles, which then contract to pump blood out of the heart. If a premature depolarization occurs in the atria, it can result in both contraction and relaxation of that chamber before its time (Figure 1). This results in an abnormal QRS complex that looks like a normal QRS followed by a smaller R wave (or even an inverted Q or R). The PR interval is normal because this beat is still conducted through the AV node and has not yet reached the ventricle.

Next, consider the physical exam findings. Patients with PAC will often have right ventricular (RV) enlargement on chest x-ray or echocardiogram. Patients with PJC are more likely to have normal RV size and function.
ECG Finally, consider your own clinical experience: If you see many cases of PH each year, you may develop a gut feeling about which patients are likely to have which disease process.

What causes a PJC?
Polycystic kidney disease (PKD) is a common inherited disorder that causes cysts to form on the kidneys and other organs. These cysts can cause kidney damage, high blood pressure, and other complications over time.
PKD often runs in families, but it isn’t caused by a single gene or passed down through the generations in a predictable manner. Instead, it’s caused by a combination of factors that increase your risk for developing PKD.
The most common type of PKD is caused by an ECG autosomal recessive inherited mutation in the PKD1 or PKD2 genes. These genes provide instructions for making proteins needed for normal kidney function. People with autosomal recessive PKD inherit two copies of the defective gene — one from each parent — and develop PKD because they lack a working copy of the protein produced by these genes.

How do you treat PJC?
There is no cure for polycystic kidney disease, but there are treatments that can help manage the symptoms.
The main goal of treatment is to slow down or stop the progression of the disease. This is done by treating high blood pressure, diabetes and other conditions associated with PJC.
If you have PJC and symptoms aren’t controlled with medication, your doctor may suggest a surgical procedure called nephron-sparing surgery. This procedure removes part of one kidney and replaces it with an artificial kidney (dialysis). It’s only recommended if you have two kidneys affected by cysts.
What is treatment for junctional rhythm?
